12. Notice under Sections 5(6), 6(3) and 7(2)(a) or (3).-

The notice under sub-section (6) of Section 5, under sub-section (3) of Section 6 or under clause (a) of sub-section (2) or sub-section (3) of Section 7, shall be issued by the assessing authority in Form-6.
